I got mine done by the dentist at "Friendly Dental". It's in Phuket, the street that runs parallel to the beach road. It's in the area where all the open-air fish restaurants open up, side-by-side and in mass. That area.

I paid 8500 baht. He made them freaky-white (a good thing). I'd do it again.

I have had teeth whitening done twice in Thailand, once at Bangkok Smile Dental (near the Furama off Sukhumvit) and early this year at the place Anthony mentioned in the Central Mall in Pattaya. Central Mall was only partially opened in Jan or Feb and so they were offering significant discounts . I took the more expensive, "latest" treatment which they explained would result in less sensitivity - don't recall any mention of it lasting for 3 years as opposed to one. But maybe they saw me for a man of bad habits who would probably want an annual touch-up. Both were fine and approximately half the price of what you would pay here.
